Please use the form below to provide information on required and recommended textbooks for your course, which will be posted to the textbook lists on the Library’s website. The textbook lists fulfill OHSU's legal requirement to provide this information under the Higher Education Opportunity Act (HEOA).

Please make one submission per textbook. Include all books needed, even if previously purchased in the cohort or program. If you need to submit many textbooks, you may wish to use our bulk submission form instead.

Note that the library will endeavor to purchase an e-book version of all textbooks whenever possible; if no e-book is available, the library will purchase a print copy and place it on course reserves.
