Bulk Textbook File Submission

Please use the form below to provide information on required and recommended textbooks for your course, which will be posted to the textbook lists on the Library’s website. The textbook lists fulfill OHSU's legal requirement to provide this information under the Higher Education Opportunity Act (HEOA).


The library will endeavor to purchase an e-book version of all textbooks whenever possible; if no e-book is available, the library will purchase a print copy and place it on course reserves.


This bulk submission form offers the option to submit information about many textbooks at once, and can be used for a single course, entire program, or entire cohort. If you only need to submit one textbook, you may wish to use our single textbook submission form instead.


Instructions:


  1. Download the template form.
  2. Fill it out for all textbooks in your course/program/cohort, including any previously purchased textbooks.
  3. Save the file, then upload and submit it below.
