PLEASE NOTE:

This form is primarily used to request a change of the existing management agent for any property that is currently in active compliance monitoring status with OHCS. It may also be used to update the management agent packet on file for any property undergoing a transfer of ownership.


Properties that are working with OHCS Production to establish an initial management agent, such as a new construction or acquisition/rehab being awarded OHCS funding for the first time, should NOT use this form. This packet should be submitted a minimum of 30 days prior to the change taking effect.

3rd party Management Agents are subject to licensing requirements by the OR Real Estate Agency per ORS Chapter 696. Certain licensing exemptions apply.


Does at least one current Management Agent Employee hold a valid OR Real Estate License? Documentation of licensure must be provided if applicable.

Please upload the following required documents, as indicated in the OHCS Management Agent Packet's "Required Attachment" list:



  • OHCS Management Agent Packet pdf (ALL fields completed, N/A for those not-applicable fields, and fully executed with appropriate signatures)
  • Written Management Plan
  • Written Property-Specific Maintenance Plan (routine and preventative)
  • Management Agreement
  • Management Organizational Chart
  • Unit Detail List (Building, Unit #, Number of Bedrooms, and Sq Footage for each unit)
  • Property lease and all applicable addenda that will be used
  • Copy of OHCS-approved Resident Services Plan narrative along with any MOUs/service agreements (if required per funding program; refer to regulatory agreements)
  • Tenant Selection Plan
  • Full AFHMP package (AFHMP document, census data, maps, copies of marketing material, documentation of Fair Housing training)
  • References (if a new agent to OHCS)
  • Real Estate License Documentation (save a copy from https://orea.elicense.micropact.com/Lookup/LicenseLookup.aspx)



Documents must be fully complete and saved in their individual respective files with clear naming conventions. Do not combine unlike documents together. Note that additional documents may be situationally required.


A $300 charge will be invoiced to the property for any management changes.


Please review your submission to ensure it is fully complete. Failure to provide ALL required documents will result in the automatic denial of the submission and request for re-submission.

If you are unable to attach all of your files due to limitations, please make a secondary submission.
