It must be completed for each pre-apprentice for each training AFTER course completion. Prior to this submission a pre-apprentice intake form MUST have been completed: Click Here
This form must be completed in its entirety as an incomplete form cannot be saved and returned to at a later time. At the end of this form always check the box for "Send me a copy of my responses." This will acknowledge submission and provide a detailed summary from a Smartsheet email. Please forward questions or request for additional information to: apprenticeship@suny.edu. For credit students, please attach enrollment report from registration system (i.e. Banner) For non-credit students, please attach attendance record signed by instructor. For all students, books and/supplies itemized receipts and detailed cost breakdown per course (Credit: tuition/fees/lab) (Non-credit: instructor/fringe) must be attached.
