Request Student Evaluation for Online Course

Please complete this request for a J2W student evaluation of an online course to be administered in your Canvas course site.


Student evaluations can only be administered in the current term Canvas course sites. If you need a student evaluation for a future term, please wait to submit your request after you have published the Canvas site for the course.


The student evaluation survey is not available for Summer courses.


Submit your request at least one week prior to the week your survey is to be administered. (Surveys run from Monday morning to the following Sunday evening. See question #9 on the request form.) Surveys administered closer to the end of the quarter seem to have higher student response rates.


If you need to invite a peer or administrator for a J1 review of a course site, please submit a Guest Access Request Form.

9. Which week of your course do you want your students to complete the Student Evaluation Survey?

If you are full-time, probationary, you can only make the survey available on Weeks 6, 7, 8 or 9. NOTE - Late Start Courses: Select the week of your course and not the week of the quarter!

*Note: Per FA contract, the Designee is the person who administers the survey. The Designee is the faculty member or other employee you designate to handle the results. The Designee cannot be you, the instructor assigned to the course being evaluated. Only the person who you select as your Designee will be able to see the students responses to the survey. Your Designee will receive the survey results via email automatically and will be provided with instructions about how to give the results to you.
