This form is designed for currently enrolled McKeown School of Education students to request approval to formally change pre- practicum or practicum field placements.

If you are changing to an employer-based field placement, please note that we are required to obtain principal approval for the placement to occur at the job site; please be prepared to provide your principal's accurate email. If you are changing to a new fellowship placement, please note that you are also required to submit a signed Fellowship Approval Form and the end of this submission.

All questions about this form should be directed to the McKeown Portal. Students will normally receive an email response to their SSU email account within 48 business hours.
