Classroom & Outdoor Space Change Request Form

Please expect a 2-3 business day turn around for requested changes. To request a new reservation for AY26, use the AY26 New Request Form, available on the Medscheduler website.

 
 

This number is located on the booking confirmation form that was sent to the requestor and lead faculty (e.g. 2023-ABCDEF). For academic courses with no confirmation number, please enter the course number.

 
 
 

NOTE: As per the SP policy, if an event is being changed or cancelled within 14 days of the scheduled date, all scheduled SPs must be paid in full for the hours that they were originally booked.

 

To request a new reservation for AY26 (dates after August 24, 2025), use the AY26 New Request Form (available on the Medscheduler Website after April 15, 2025). If you already have a confirmation for AY26 dates that you would like to make changes to, continue with this form.

 

Enter your additional AY26 autumn/winter date(s), followed by the start and finish time. If you are only requesting changes to rooms, resources or set up/take down time and not adding dates, enter "N/A" and list those changes in the Comments section below.

 

Enter any relevant additional information about the change. For example: change of start/end time, add setup/takedown time, update headcount, add/remove a resource, etc.
