HR Personnel File Request Form
This form should be used whenever an current or former employee, within the retention guidelines, requests to review their personnel file. This form should also be used when an authorized City of Detroit agency requires a review of a current or former employee's personnel file. Due to COVID-19, file reviews must be scheduled in advance and will be conducted via the Zoom conferencing app. A valid Government photo I.D. must be uploaded to this form for processing. Personnel files will be retrieved on a weekly basis. Please allow 5-7 business days to process the request.
