Methods to apply for Re-accreditation Version 1 re-accreditation at the same level: Version 1 applicants are institutions re-accrediting at the same level as they were initially accredited in and who have little or no changed in their organisation and laboratory. For example, if they have achieved Advanced TTE and Standard TOE and are seeking accreditation in Advanced TTE and Standard TOE. The institution is requested to complete the re-accreditation form which will be shared with them once step 4 of the application process is complete. Version 2 re-accreditation at a different level: Version 2 applicants are institutions re-accrediting at a different level to the one they were initially accredited in and/or have major changes in their organisation or laboratory. For example, an institution who has achieved Standard TTE and Standard SE seeking accreditation in Advanced TTE and Advanced SE. These institutions will be requested to complete the full application form as they did for the initial accreditation process. This information will be shared once step 4 of the application process is complete.

The information collected during the re-accreditation of laboratory process is subject to data processing to finalize your certification programme. Without this information, we will be unable to properly process your laboratory re-accreditation submission. The recipients of the data are ESC Staff who process the forms as well as national societies and committee members. Data collected will be kept for a maximum of 5 years after your last transaction or activity with us after which time your My ESC account will be closed These data will be stored in software application for collaboration (eg: Smartsheet) and CRM service providers (eg Salesforce) for account management, email service providers (eg: Salesforce Marketing Cloud) to contact you and others according to your specific actions within your account. In accordance with the chapter 3 of the European Regulation 2016/679 with regards to data protection, you have the right to request from ESC, access to and rectification or erasure of your personal data or restriction of processing concerning your data or to object to processing as well as the right to data portability.
