Non-Construction Subcontractor LOTO Request
You have indicated that your company would like to perform Lockout/Tagout (LOTO) for the purposes of working at Berkeley Lab. The following steps will need to be completed for this to occur: 1. Non-Construction Subcontractors must have an established Subcontractor Job Hazards Analysis (sJHA) in accordance with the Berkeley Lab sJHA Process – Subcontractor Job Hazards Analysis policy. This process will trigger the requirement for a Subcontractor LOTO Permit. Your LBNL host can guide you thru this process. 2. Unless one of the exemptions below apply, your company must submit your company LOTO Plan for LBNL to keep on file for future reference, if needed. See section titled "Company Safety Information" below. Regulatory requirement [1910.147(f)(2)(i)]. 3. Your company must provide documentation that your employees have been trained as LOTO Authorized Employees. See section titled "Employee Documentation" below. Regulatory requirement [1910.147(c)(7)(i)] 4. Each employee applying a lock and tag must be familiar with LBNL's LOTO Program. This on-line training is to familiarize them with this process. Each employee must complete the following training (EHS0379): http://www2.lbl.gov/ehs/training/webcourses/EHS0379/ (if this training is watched at one time by several employees, please write names of employees and send the list to Kmjohnson@lbl.gov for credit for all) Regulatory requirement [1910.147(f)(2)(ii)] If your LOTO activity involves electrical equipment above 50V AC (or 100V DC), a Qualified Electrical Worker (QEW) will be required to perform Zero Voltage Verification (ZVV), test, troubleshoot, inspect or modify the equipment.
