SEPTEMBER 2021 UPDATE - Wildfires around the Willamette Valley and recent weather have combined to bring severe smoke to the area. If you are reporting a wildfire smoke related concern, please provide that information in the "Issue Type" field in your report as indicated. Please contact EHS at (541) 346-3192 prior to building closure due to wildfire smoke. Please fill out the form below to report an IAQ issue. EHS will respond as quickly as possible. If this is a life threatening emergency, please call 9-1-1 right away! If you are reporting a natural gas odor or an unusual strong chemical odor, please call UOPD at 6-2919 and EHS at 6-3192 immediately.

Emergency (Call UOPD at 6-2919 and EHS at 6-3192 immediately) - Natural gas odor, overpowering chemical odor, situation that may be immediately dangerous to life and health. Wildfire Smoke Urgent - Issue is wildfire smoke related and has mild physical / allergy / irritation effects. Wildfire Smoke Nuisance - Issue is wildfire smoke related and is annoying, but does not affect me physically. Urgent - Issue has mild physical / allergy / irritation affects. Nuisance - Issue is annoying, but does not affect me physically. Observation - Issue produces no noticeable physical affects or irritation.
