Desired Date of Completion

Please indicate when you will need the survey to be completed. Please note that Environmental Health and Safety will endeavor to meet all deadlines to accommodate your work, but in some cases a discussion about completion date may be discussed. A typical turnaround for bulk sample collection (1 - 5 samples) is 1 - 2 week, a small survey approximately 3 weeks, and a large survey can extend for a month or longer.
