If you know the internal project name, please enter it here

IF the product is FCS'd, please use the device PID as you see in "show version" or "show hardware".

Device System Object Id (sysObjectId)*

If you have device access, you may use SNMP Walk on system table to get us the system object id. This is very crucial for us to make sure we have the right device to be supported.

Device Family:*

Use the right categorization to enable us to quickly support this device. If we already know the device family, it will be easy to support the new device.
