This form is used to record important information that Nesbitt Realty will use to manage your property.

The street address (and unit number if any) of the property being managed.

We will send monthly notices and emergency notices to this email.

Who should we contact about HOA/POA issues?

If so, please explain what new residents need to do.

Please list any community amenities: pool, fitness, etc.

Please list number of spaces and location of spaces. If spaces are labeled/numbered please put the parking label in paranthesis. Example: 1 space, Garage P2, (44)

Does your rental have a keyed mail box? Please describe the location and box number.

Describe any lawn maintenance that the tenant must maintain. If you have a lawn service, please put the company name and contact information.

Please describe the location and access to any extra storage.

Who removes the trash at your rental?

If the trash is removed by a private contractor, please list their name and phone number.

List any other tenant responsibilities to be included in lease such as swimming pool or hot tub
maintenance, hardwood floor protection, septic field switching, etc.

Please list any maintenance contracts. Include contact information for the contractor.

Please list any warranties for the property. Include contact information for the warrantor.

Please state your insurance company and policy number.

Do you have any pest control contracts? If so, please list the company and their contact information.

If so, please state the name and contact information for the company.

Please list the utilies that the tenant must set up.

If so, please list these repairs and the contractor's you are working with.
