MSD Equipment Service Requests

The purpose of this online form is to request service of electrical or electronic equipment through the MSD Division Office. When a request is entered, it will be triaged by MSD EH&S personnel and then the appropriate individuals will be called in to assist. In general, equipment will not be taken out of service until inspection, repair, or modification is ready to be completed.

No charges will be made until scope of request is verified and the cost is approved by the requestor.

List the device name; include make or model if appropriate.

List the building and room where equipment is located (i.e., 66-250)
