Sustainability Assessment

Pn2 is committed to helping the state teams maintain the forward momentum they have made over the last couple of years on their goals and strategic plan. Sustainability is an elusive concept, especially when there are so many factors to consider when planning the next step. To encourage sustainability with your goals and strategic plan, we are offering an online assessment to help you prioritize your team efforts as you continue the great work on your current and or new goals.

There are eight domains that can influence a program’s capacity for sustainability. Seven of the eight domains are listed with discussion points and a rating scale. The final domain, Strategic Planning, does not have a rating scale associated with it. Instead, we have added a couple of questions to assist with your team’s continuous strategic planning.

Please discuss each domain as a team, using the discussion topics to help focus your conversations and then identify your rating on the scale.




Definition:
Having a supportive internal and external climate for your program.

Discussion points:
• Discuss environmental factors that may assist or may get in the way of your team making continuous progress.
• Discuss two or three environmental barriers that may get in the way of your team making continuous progress with your goals and or plan.

Your rating:
Consider the current condition of your team’s Environmental Supports. How established are the Environmental Supports in your team? Assign a rating using the following scale:



Definition:
Establishing a consistent financial base for your program.

Discussion points:
• Think about two or three funding sources that will remain stable and consistent as you continue to move forward with your goals and or plans.
• Think about two or three funding sources that are not stable and that may interfere with the forward momentum of your goals and or plan.

Your rating:
Consider the current condition of your team’s Funding Stability. How established is the Funding Stability for your team? Assign a rating using the following scale:



Definition:
Cultivating connections between your program and its stakeholders.

Discussion points:
• Identify two or three partnerships that have been beneficial and will continue to support your goals and or plan as you move forward
• Identify two or three partnerships that could be helpful to your goals and plan as you move forward.

Your rating:
Consider the current condition of your team’s Partnerships. How established are the Partnerships in your team? Assign a rating using the following scale:



Definition:
Having the internal support and resources needed to effectively manage your program.

Discussion points:
• Discuss the capabilities, knowledge, or resources on your team that will continue to support your plan and or goals.
• Discuss the two or three capabilities, knowledge areas, or resources that your team could use or needs to support your goals and or plans as you move forward.

Your rating:
Consider the current condition of your team’s Organizational Capacity. How established is the Organizational Capacity on your team? Assign a rating using the following scale:



Definition:
Assessing your program to inform planning and document results.

Discussion points:
• Discuss two or three areas of evaluation data you have collected to measure the progress of your goals and or plan.
• Discuss two or three areas of evaluation data that you still need to collect to continue support and funding of your goals and or plan.

Your rating:
Consider the current condition of your team’s Program Evaluation. How established are the Program Evaluation protocols in your team? Assign a rating using the following scale:



Definition:
Taking actions that adapt your program to ensure its ongoing effectiveness.

Discussion points:
• Have you had to adapt your goals and or plan? If so, what did you adapt?
• Discuss two or three areas of your goals or plan that need to be adapted in order to move the plan forward.

Your rating:
Consider the current condition of your team’s Program Adaptation. How established is the Program Adaptation in your team? Assign a rating using the following scale:



Definition:
Strategic communication with stakeholders and the public about your program.

Discussion points:
• Discuss two or three areas of communication or marketing that you have used to disseminate information about your goals and plan.
• Discuss two or three components of your goals and or plans that you think need more shared information or dialogue with your stakeholders.

Your rating:
Consider the current condition of your team’s Communications. How established are the Communications in your team? Assign a rating using the following scale:



Strategic planning is the glue that holds sustainability efforts together. Without a strategic direction and long-term goals, programs find themselves only reacting to day-to-day demands. Strategic planning combines elements of all of the sustainability domains into an outcome-oriented plan. Planning also ensures that the program is well aligned with the larger external and organizational environment.

Keeping your ratings in mind, discuss how you will use this information to sustain your team’s work. Please complete the following questions with your team:
