If you've arrived at this form, delivery didn't go as expected. So, let's take time to document what happened clearly. Next, we'll devise a plan and find a way to make things right!


Please do not sign the Proof of Delivery (POD) without inspecting your order; otherwise, we (Texas Metal Works and you) cannot file a successful claim.


Examine the pallet, boxes, table legs, top and walk around the entire delivery. We suggest taking pictures of your delivery from all angles before the driver leaves.


Make sure to submit

  • Copy of delivery receipt
  • Photos of freight damage (if applicable)
  • Copy of inspection report (if available)
