Dive Gear request

Please give your full name (first and last name).

Please provide your KAUST email.

It is critical to ensure you provide your email to receive notifications about your request.

Please add your mobile number (it has to be a KSA number).

Please select your department.

Please add your PI's full name (first and last name).

Please input your approved pocket ID. Ensure the pocket ID is registered in the Core Labs Infinity system. Any requests with an invalid pocket ID will be rejected.

If you have a submitted field work please enter its request number.

Dive Gear Request will not be approved without correct Dive Plan Number provided.

Please input your desired start date. All bookings must be made at a minimum 2 working days in advance.

Requests made in a short notice (<2 working days) will be rejected.

Please provide the quantity.
