This form is to be filled out by schools in Clark County with active school cafeteria waste reduction programs, such as sort tables and share tables. The purpose of this yearly check-in is to provide ongoing support to schools to assess needs and offer additional resources. A portion of this survey is also conducted for data gathering purposes to help us track our green schools' progress and for metrics for county-wide waste reduction.

In order to fill this out accurately, we suggest observing one full lunch period and speaking with the custodian (only if the custodian is not able to fill out this form).
