IT Assessment Request Form

This form is the official way to submit technology requests. It allows customers to have their needs reviewed by IT and receive necessary resources or services. We want departments to proactively submit requests when considering new technologies to ensure security and integrity of the city's infrastructure. Failing to do so could expose us to security vulnerabilities and threats.

Enterprise: Does this request involve 3 or more departments?

Project Scoped: Is there a Scope of Work defined

Council Initiative: Does this request include any of the following: Workforce Housing Sustainability, COVID Response, Community Safety, Economic Development, Downtown Vitality, Assessment of Community Assistance, Develop Equity Policy
