External Fellowship Support Request Form

This form is used to request consideration for the University of Oregon to provide institutional support to cover differentials associated with tuition/insurance/stipend/fees for individual fellowships not automatically approved:

Representatives from OVPRI, the Division of Graduate Studies, and the applicant's school/college will review this request and aim to provide a final decision within 5 business days of receipt.


You must submit this form at least 10 business days prior to the sponsor deadline to guarantee institutional approval of this request prior to submission.

Note this should be the graduate student or postdoc, not the faculty advisor.

Please note that students should not directly submit this request, but rather, should work with their DGA to submit.

Please provide the name of the funding agency or organization.

Please provide a brief description of the support provided by the award (e.g., stipend, healthcare, research supplies, travel).

Please indicate the years for which tuition is requested and the number of terms per year.

How many years of GE funding was the student guaranteed upon matriculation into their graduate program?

What year is the student in their current graduate program?

Please estimate the total requested financial support required for differentials in tuition/insurance/fees/etc.
