I understand that the Planning Association of Washington (PAW) has scheduled an in-person educational program to be held on November 15-16, 2021 at Campbell's Resort. Our attendee's safety is a top priority for PAW. For our 2021 Annual Conference, PAW will be requiring attendees to bring their proof of COVID-19 vaccination cards (at least 2 weeks after final dose) or proof of a negative diagnostic test within 72 hours upon registration check in, Monday morning. By signing this Release and Waiver of Claims, I acknowledge to provide this documentation and agree as follows: • By participating in or attending the Program, I risk becoming exposed to or infected with COVID-19 (a/k/a the novel coronavirus) as a result of my actions, omissions, or negligence, or those of other attendees or participants, including without limitation, PAW volunteers and other members. •COVID-19 is extremely contagious, and exposure to infection can result from person-to-person contact, including but not limited to breathing, coughing, talking, interacting with other individuals, and touching surfaces, among other methods. • Attending the Annual Conference could increase my risk of contracting COVID-19. • PAW is not responsible for Campbell's COVID-19 mitigation efforts (or lack thereof) or any negligence by Campbell's or its staff relative to such precautions. • The Annual Conference been made available by PAW for educational purposes only. I acknowledge and agree that it is my obligation to follow any recommended health and safety measures, including but not limited to: (i) Avoid person-to-person contact such as handshakes or hugs; (ii) Maintain a minimum distance of six feet from others to allow for “social distancing;” (iii) Wear a face mask or covering, especially whenever social distancing cannot be maintained; and (iv) Wash hands frequently throughout the day
