EANS I and II Disposition Notification Form

The purpose of this document is to provide instructions and guidance for the disposal of items purchased through EANS. If you reported during monitoring that you will continue to use the items purchased with EANS I and EANS II funds, please use this form when you are ready to dispose of those items.

Please include quantity and why you are disposing of the items

Please provide the fair market value of all items that you wish to dispose. Fair market value is the average of what the items may cost in the open market.

Items No Longer Needed for the Intended Purpose

        A public agency must keep title to and exercise continuing administrative control of all equipment and supplies that the subgrantee acquires with program funds. 34 CFR 76.661(a). Upon receipt of this request, CDE will determine if the items can be disposed of. When the non-public school has received approval for the disposition of items, CDE will contact them with further instructions for drop off. Items that are no longer in use, not damaged, and above an aggerated cost of $5000 will need to be dropped off at 201 E Colfax Ave, Denver CO 80202.



        School officials will need to request a drop off day/time by emailing eansapplications@cde.state.co.us before returning items to CDE. All disposition information should be maintained on the inventory for a minimum of three years after final disposition. 2 eCFR ยง 200.334
