Report a concern to Compliance and Civil Rights (CCR)

This form can be used to report concerns of discrimination, discriminatory harassment, sexual harassment, sexual assault, dating violence, domestic violence, stalking, and hate crimes. In most cases, CCR will reach out with information about resources, reporting options, and an invitation to meet and discuss your concerns. This intake consultation is an opportunity for you to learn about your options, ask questions, and receive information on supportive measures that may be available to you.
