COC/LHC Letter of Support Request Form

Please complete the information below to submit your request for support from the Continuum of Care (COC) and Local Homeless Council (LHC). The Salt Lake Valley Coalition to End Homelessness (SLVCEH) fulfills the role of the COC/LHC.
