Request for Used Furniture, Request for Furniture Transfer, and Equipment Transfer Document
Request for used furniture: Use this form when used furniture is requested for your site or department.
Request for furniture transfer: Use this form when furniture is to be moved from one site/department to another site/department. Or if no longer needed the items may go back to the distribution center. These items must be in good condition.
Equipment transfer document: Use this form when equipment and/ or furniture that has a CTag or a manufacturer serial number and is no longer of use at your site or department. These items are beyond economical repair or obsolete.
*A printed copy of your request must be placed with the equipment and/or furniture in order for distribution to pick up.
