Report of Incident or Non-Work Related Injury

This form applies to any incidents that may need to be reported in a timely manner to University insurance AND for all injuries that were NOT incurred during employment with the University.

Injuries incurred in the course of employment of faculty, staff or student workers fall under the Workers' Compensation Policy and should be reported on the Supervisor's Report of Injury form.

Alumni Advisers should report incidents and injuries on the Alumni Adviser Incident Report form.
