Only parents or legal guardians are authorized to complete the online checkout form for each participant. Please ensure that this form is submitted at least 48 hours prior to the requested checkout date to allow for approval by the Stanford Pre-Collegiate Studies staff. Be aware that all requests are subject to approval, and the program reserves the right to deny any request if there is a scheduling conflict.

In case of a family emergency or unforeseen circumstances requiring a participant to leave the program temporarily without prior approval, please refrain from submitting the form. Instead, promptly call (ARD Duty Phone Number) to inform Stanford Pre-Collegiate Studies staff about the participant’s departure and expected return.

I acknowledge that all requests must receive prior approval from Stanford Pre-Collegiate Studies. Requests may be denied under the following circumstances: if the approved adult is not listed in CampDoc, if the checkout duration exceeds the three-hour limit, or if the request conflicts with scheduled program activities.
