The Online MPH Program has two mandatory on-campus summer visits, requiring students to be on campus for 4 days in their first summer and 6 days in their second summer. Students must be present during all required sessions of both summer visits. The summer visits fulfill the program's residency requirement.

Students should complete this form to request an attendance exception due to extenuating circumstances outside their control (e.g. medical emergency, family emergency, visa issues). Completing the form does not guarantee approval. Students will be notified via email about the decision of their request.
