If you are interested in scheduling an appointment for your child(ren) at one of Nationwide Children's Hospital’s School-Based Health Centers, please complete this form. A hospital staff member will be in touch with more details.

We offer Primary Care Services including checkups, sick visits, physicals, vaccines and more. We can also diagnose and treat anxiety, depression and ADHD.

Please submit one form per child. The health centers are open to all children and community members, ages 0-21. We accept Medicaid and other forms of insurance. Financial assistance is also available.
