Faculty may use this form to submit trainings completed outside of PSOM/ITMAT Ed Offered Trainings. After review and approval, credit for the training will be added to your Workday Learning record.

If you competed a PSOM/ITMAT Ed Training, do not submit this form. Your training record is automatically captured and added to Workday Learning.
