On Leave Request Form

The graduate student On Leave Request form must be initiated by the student to petition for a leave of absence from the Graduate School.


  1. Review the section on continuous enrollment in the Graduate Catalog
  2. Communicate your intentions with your advisor
  3. List the email address for your advisory committee chair or graduate coordinator for approval.


The Graduate School will review and provide the final decision once the required signatures (including the Bursar's Office and Student Financial Aid) have been obtained. Contact Debbi Clements (debbi.clements@maine.edu) with any questions.

You must communicate with your supervisor if you plan on being on leave. Students are not permitted to maintain university funding while taking a leave of absence.

I acknowledge that I will be out of contact with the University of Maine graduate faculty and facilities for the period indicated. In order to maintain my place within the Graduate School, I petition for approval of the above request.
