The purpose of this form is to provide a structured process for requesting a training from the Training and Education Team at the MaineHealth Center for Tobacco Independence that is not part of the regular scheduled trainings.

• The Training and Education Team will need at least 8 week prior to your event for planning and preparation.
• You will receive a response from the Training and Education Team within 5 business days of your request.
• Please fill out every question below and check off the box at the end of the form to get a copy of your training request.
