Is the asset currently not in service due to this request?
Please enter the bus fleet #. Examples include MOL 2267, BCT 9, DETE2, PVTA 1414. Production bus names are acceptable, and look like Miami2 3.
Please enter the VIN of the asset; at a minimum the last 6 digits of the VIN are required. Entering the full VIN is also acceptable.
If the request isn't related to a specific VIN, put "Not Vehicle Specific".
Retrieve and document any fault codes from Phoenix Diagnostic Tool prior to submitting request. If no codes do not exist please enter 0.
Please enter the system that is applicable to the request. Examples: Prodrive, Duopower, Powertrain, Battery, Controllers, etc
Select one of the assigned RSM's. Do NOT self populate. Do NOT Enter state. Recently Service Managers regions havedchanged. If your previous region was Northwest, Southwest Central or Midwest select Ramon for West Coast support. If your region was Northwest or Southwest please select Luis Baglin for East Coast support.
Date failure last occured. Critical for remote data review.
Appx time of failure. Critical for remote data review.
