Evaluation - Health Network: A Care Coordination Program for Mobile Patients

The purpose of this session was to familiarize the attendees with the importance of bridge case management for continuity of care and reduction in patient loss to follow up for migrant populations. This event was designed for all primary care providers involved in the initial care for mobile and homeless populations. Upon the completion of the activity participants should have a better understanding of the role bridge case management plays in continuity of care, be better prepared to identify migrant and homeless populations and gain the tools to help participants better address barriers to care, continuity of care, and issues in loss to follow up.
