Capital Project Request Form

All new project requests or changes to existing IT systems/facilities should be submitted by using the web form below. Once completed, the proposal will be sent to the appropriate division head for review. Academic Division submissions will be sent to the Provost’s Office. After review and approval from the division head/Provost office, a representative will be in touch to review the project in more detail.
**Please note: The deadline for on-line submission of new Facilities capital budget requests is Friday, November 30, 2018. **

Provide a short name for the project. This name will be used to describe the project throughout the process (example, Photo ID Integration).

Capital - Creating new capabilities, program, or spaces
Programmatic - Enhancing or expanding existing capabilities, program, or spaces
Stewardship - Replacing/repairing existing capabilities, program, or spaces

Please indicate the priority of your project.

Provide a desired deadline.

Please provide a short paragraph that describes the project in general. It should clearly and simply outline the primary objective(s) of the project. Include details such as: why is the project is needed; is it required to comply with a new law/safety/board mandate; will it improve/enhance a business process; will it improve/enhance teaching, learning, and research. You should also describe the expected benefits this project will deliver, and what may/will happen if the project is not completed.

List all individuals, departments and/or groups that may be involved in the project.

Submit any additional relevant details about the project (e.g. known vendors or contractors that provide the required capabilities, knowledge of similar capabilities at other institutions, systems/software/hardware, estimated costs, furnishings, etc.).
