SHIP - Quarterly Reporting Submission Form (for Subrecipients)

Please review your subrecipeint contract (subward) for details and specifics on the list of anticipated reports that are required to be submitted during the term of each subrecipient supportive housing contract.


This form will mostly be used to submit the required quarterly reports, as noted in Section 4 of the subrecipient agreement.

The subsequent quarterly reports will cover one calendar quarter and must be submitted to Treasury within 30 calendar days after the end of each calendar quarter. Quarterly reports are not due concurrently with applicable annual reports. Please refer to Section 4.1 of the subaward contract for quarterly report due dates.

Expenditure Requirements and Project Status

Per the subaward, as stated in the LFRF Compliance and Reporting Guidance, all LFRF funds must be obligated no later than December 31, 2024.

Project descriptions must describe the project in sufficient detail to provide understanding of the major activities that will occur, and will be required to be between 50 and 250 words. Projects should be defined to include only closely related activities directed toward a common purpose.

Infrastructure Projects: Wage and Safety Compliance Reporting

Provide the following:

The number of employees of contractors and sub-contractors working on the project;

▪ The number of employees on the project hired directly and hired through a third party;

▪ The wages and benefits of workers on the project by classification; and

▪ Whether those wages are at rates less than those prevailing.

Travis County must maintain sufficient records to substantiate this information upon request.
