Foreign Travel Submission


This form should be used to submit Authorized Travel for Employees to Risk Management for scheduling insurance coverage under the University's Foreign Liability Policy. An approved TA number is required.


Certain destination countries may require additional approval and documentation for travel to be scheduled under the Foreign Liability Policy.


Export Control will be notified of travel to destination countries that are under restrictions with the Office of Foreign Asset Control (OFAC) or for compliance with other applicable state or federal regulations.


If completing on behalf of departmental staff, the submitter must notify staff of additional steps in registering technology with Export Control, completing additional documents as required, and to review and update emergency contact information as it applies to their travel.


Students, not employed by the University, but who are traveling abroad under the auspices of Boise State University, must contact the Center for Global Education prior to travel to ensure the mandatory Student Travel Accident insurance is purchased and to ensure all safety and medical issues relating to foreign travel are properly addressed. This includes but is not limited to student travel related to graduate studies, field work, research, or other educational purposes.

State Department Travel Advisories are assigned to countries. Please review the State Departments website and indicate the current level assigned to each of your country(ies) of destination.


If you have more destinations than indicated, please submit a separate document at the bottom of the form with the State Department Country Advisory levels listed for the additional destinations.


Level 1: Exercise normal precautions. This indicates the least risky places to travel to, but the State Department cautions that "there is some risk in any international travel."


Level 2: Exercise increased caution. Travelers should be aware of higher safety and security risks in their destination, and that conditions can change at any time.


Level 3: Reconsider travel. Serious safety and security risks exist and conditions can change at any time. The State Department provides more specific guidance for travelers in Level 3 areas.


Level 4: Do not travel. Level 4 countries represent the highest risk of life-threatening danger and little to no ability for the U.S. government to assist when an emergency occurs. The State Department advises travelers to avoid Level 4 countries, that Americans in those countries leave as soon as it's safe to do so, and to write a will and leave DNA samples before travel in case of worst-case scenarios.
