Roster Freeze Change Request

****Please Note: This form must be filled out for each Change Request. So if you have 5 players to add or remove then this form needs to be completed 5 times for each player.****


This form is for teams to use that wish to make changes to their frozen roster for Kansas State Cup, Kansas Presidents Cup or Kansas Jr. State Cup according to the Rules of Competition Section VIII Roster Freeze Dates Part C & D which states:


C) The second roster freeze date is after the Cup Draw has taken place as mentioned in Section XXII. Part D. The date has been set by Kansas Youth Soccer anywhere between 5-15 days prior to the start of the event. This date is for teams to make up to Five (5) changes to their frozen roster as long as the team making the changes still have a 51% roster continuity from league play, and the top group positioned teams meet the requirement in Section VIII. B. All changes to the roster before the second roster freeze date must be submitted too & approved by the Cup Committee. All required check in documentation (See Section XVI Part F) is needed at the time of the submission for the roster change/addition to be approved.


G) State/Presidents Cup teams advancing to the Midwest Cup events, or the National Cup events may add or change club pass players in accordance with the rules and policies set forth by US Youth Soccer and the Midwest Region rules that govern participation in those events.

What counts as a Roster Change?

The following are examples of what teams may do and what counts as one (1) change.


Even Swap: A team adds a player and removes a player because all 22 roster spots are full will count as only one change. (16 roster spots for Jr. State Cup)


Addition only: A team has 4 roster spots open and you're unsure about one player due to injury. Leave them on roster in case they are cleared to play since you have the roster positions open and up to 5 changes still.


Removal only: If player completely quits or leaves your team. You may remove the player or players and it would NOT count as a change because you have not added any other players. However if the removal of the player would put you below the 51% roster continuity from league to tournament event then the request will be declined and that player has to stay on the teams roster and within that club until after the deadline.


Needs to be a coach, manager or club registrar.

You will receive an email stating if the request for roster change was approved or declined before the deadline.

Required Player Documentation at time of Request

********************************************************** For Players Within Your Own Club: Two documents will be needed for the player to be added to a state tournament roster if approved. - US Youth Soccer Medical Waivers ---> Must be current but does not need to be notarized ---> We do NOT accept US Club waivers. Proof of Age Documents (Accepted Documents below) ---> Birth Certificate ---> Passport ---> Current Drivers License ---> Uniformed Service ID & Privilege Card ---> Alien Registration Card ---> Unexpired Federal, State, or Local Govt. ID Card ********************************************************** For players being added outside your club & out of state: These players require additional paperwork that needs to be submitted along with the Medical Waiver and Proof of Age. Out of state players need to complete the Interstate Permission Form and get approval from that players home state youth association. Out of club players but within Kansas need to either be released from their current primary club or complete a dual roster form that would make them secondary to the club team they were on and primary for the club team playing in the state tournament event. ********************************************************** - PLEASE TITLE EACH UPLOAD WITH THE TYPE OF DOCUMENTS THAT ARE INCLUDED. Whats Next?

1) KSYSA will send email to the submitter if request was approved or declined before the deadline.


2) Upon receiving approval you may contact your club registrar to make these changes but if not then KSYSA will make the changes to the state tournament roster.


3) Once changes are made please contact your club registrar to get the players card printed and laminated.


4) Take that USYS ID Card to each game and enjoy the tournament event.
