OVPRI On-Campus Research

In response to Executive Orders from the State of Oregon, and in order to ensure the safety and wellbeing of our faculty, staff, and students engaged in maintaining essential research activities, any person coming to the UO campus is required to log their on-campus research activity using this form. Please complete this form prior to any visit to campus. OVPRI will use these responses to coordinate effectively with our campus partners (Environmental Health and Safety, Safety and Risk Services, UOPD, etc.) to help protect the health and safety of our community and facilities while maintaining critical research functions. UPDATED: Effective June 15, 2020, face coverings will be required in UO owned and operated buildings EXCEPT when individuals are alone in an enclosed room. At this time, employees may use their own face covering (i.e., cloth or paper mask, or other cloth accessory that, when in place, covers both the nose and mouth) or pick up a disposable mask via the ID card office on the ground level of the EMU (west entrance (by amphitheater) is open Monday- Friday, 9 AM to 5 PM). UO is in the process of procuring a large quantity of high-quality cloth face coverings. We anticipate making two available to every employee and student beginning in August.
