This request form will be used to initiate all Facilities and IT projects in an effort to collect pertinent data necessary for decision and resource planning. The use of this system will:


  • Ensure project alignment with University Goals
  • Optimize internal resources of Facilities, IT and the University as a whole
  • Prioritize initiatives based on budget and human resources
  • Reduce the quantity of redundant projects
  • Help ensure high-quality results


Upon submitting this form, your request will be sent to the Facilities and IT Project Management team for review. Within 5 business days, a team member will be in touch with next steps.


To expedite the review of your request, please provide as much detail as possible and include all required documentation in your submission.

Sponsors and Project Leaders

This individual will be heavily involved in the successful execution of this project. They must be available for meetings, provide insight into current operations and advise on desired outcomes.


*The Primary Project Owner can NOT be the same individual as the Project Sponsor.

 

This individual(s) should be able to perform the duties of the primary project owner, either in conjunction with or in the absence of said primary project owner.


*The Backup Project Owner can NOT be the same individual as the Primary Project Owner or the Project Sponsor.
