Ward 8 Healthy Living Micro-Grant Application
BACKGROUND AND PURPOSE Giant Food is the leading regional food retailer in the Washington Metropolitan Area, with 164 supermarkets in Virginia, Maryland, Delaware, and the District of Columbia. Giant Food is committed to supporting the various communities it serves through education and resources that support healthy living. In 2018, Giant Food set out to do its part in addressing poor health outcomes in Ward 8 by focusing on improving the KNOWLEDGE of healthy food and its benefits, the AFFORDABILITY of food to the ward, and ACCESS to healthy food. Through the work of the Ward 8 Healthy Living Microgrant Program recipients and other community partners, Giant looks forward to continuing to serve as a positive neighbor to the Ward 8 community. FUNDING PILLARS The Ward 8 Healthy Living Microgrant Program seeks to support a wide variety of programs, reflecting the diverse passions and needs of the Ward 8 community. The microgrants, ranging in support level, will fund projects through which the following outcome(s) will be achieved: 1. NUTRITION KNOWLEDGE: Improving knowledge of food and its benefits to health. 2. HEALTHY FOOD ACCESS: Improving access to healthy food (physically, logistically, transport, etc.) 3. FINANCIAL LITERACY: Improving financial education and management. 4. HEALTHY LIVING: Improving access to movement and other stress-reducing, mindful living activities that promote physical and emotional health. Applicants may request funding to support new or existing programs, capital projects, equipment purchases, general operations and capacity building. Example projects include but are not limited to: • Community Events (grocery support, in-kind donations) • Cooking classes • Physical activity/ movement programs • After-school wellness programs • Financial education programming • Subsidizing the donation and sale of healthy food items in non-grocery retail locations Giant Food is seeking qualified community partners who want to address health disparities in their community, understand the needs of the Ward 8 community and have expertise or experience within a specific Funding Pillar. Applicants may request up to $2,500 . Only one application per organization per year (annual funding is not guaranteed). Larger requests must be submitted through Giant Food’s Charitable Giving Request Page. (https://giantfood.com/pages/donations) PROGRAM ACTIVATION Programs funded by the micro-grant should take place within 6-months of fund distribution. Approved projects may be subject to further review post-proposal approval. The following are the primary activities expected in the grant arrangement: FOOD FUNDING (If applicable) All funds provided to support the purchase of food and beverage, should be used to purchase such items from Giant Food. CONGRESS HEIGHTS WELLNESS SPACE USAGE It is highly encouraged for Applications to include a class or other program that will utilize the Congress Heights Wellness Space, located inside of the Giant Food at 1535 Alabama Ave SE, Washington, DC 20032. PROGRAM REPORTING Grant evaluation reports are due 6-weeks after program conclusion (no longer than 6-months after fund distribution). Grant reports should include an impact statement and photographs (with release for Giant Food to use for internal and external communications) of the project that Giant will utilize. See Grant Report Form for additional evaluation requirements. If a program report is not returned, your organization will not be able to apply for future funding support from Giant Food.
