Badge Request Form

An ID badge is issued to an employee or affiliate once they have been cleared to work.


The person receiving the badge is responsible for ensuring that the badge and access granted will be used in accordance with UCSF policy.


Employees, vendors and contractors must have a badge while on UCSF property. While at UCSF locations, badges must be visible and above the waist, unless work prohibits for safety reasons. Individuals must badge in at their primary work location any time they report to work and should not use their badge to allow access to others.


Employees, vendors and contractors are required to immediately report if a badge is lost or stolen. Badges are the sole property of UCSF and must be returned upon request, termination or the end of active student status.

For new hires, please provide your personal email address. For existing employees, please provide your UCSF email address.

 
 

If you are a new hire, this date should match the date on your offer letter.

Please upload a photo to use for the identification badge. The photo should be taken in front of a neutral color background, with good lighting on your face. Please also ensure that your entire face is visible.


Photos must be submitted in an image format type (e.g. JPEG) or as a PDF. Pictures of a photo ID are not appropriate.
