Thank you for choosing Memorial Health in Central Illinois for your job shadowing needs. This form is for anyone interested in observing at any Memorial Health location. This request is for observational experiences only. There will be no hands-on activity allowed during job shadowing. You must be at least 16 years of age to job shadow (18 years of age in some areas).

Requests must be submitted at least 30 days prior to the date you are requesting to observe.

If requests are submitted without ample time for review and completion of requirements, we cannot guarantee you will be able to observe on your chosen date.
