Group Registration

This form is intended only for group registrations of 5 or more people from the same agency, institution, or group practice.


The group registration rate is $50 per person, including CEU's.


Please do not complete this form for registrations of less than 5 people or 5 people from different practice settings.

Group Registration

You will be contacted to follow up on your group registration to collect individual registrant information from your group. Please prepared to provide:


  1. Registrant first name
  2. Registrant last name
  3. Registrant email address
  4. Registrant phone number
  5. Registrant credential/license type
  6. Registrant seeking CEU's (yes/no)
  7. Registrant experience writing letters of support



The Transgender Health Program will contact the person completing this form to collect registrant information and payment.
