STEP 2: Check all required statements below

Application Confirmation

I understand that completion of this form and payment of a deposit constitute an application and that my participation is not guaranteed until confirmed by IKAR. If IKAR is unable to confirm my participation, my deposit will be refunded in full.*

Ride for the Living

I understand that participation in the Ride for the Living includes a best-efforts fundraising component (goal: $1,000 per rider, $500 per non-rider, $1,500 per couple, and $1,800 per family) and am prepared to encourage friends and family to support the effort.*

Transaction Fees

I understand that I am responsible for any credit card transaction fees and/or bank wire fees related to payments for this trip, and that if I make payments using my credit card, I will check the box to add the transaction fees to my payments.*

Price Changes and Payment Schedule

I understand that prices are subject to change, for reasons including but not limited to exchange rates and updated hotel & transportation pricing, and I agree to make trip payments consistent with the published schedule of deadlines (currently 25% by December 24, 2021; 25% by February 28, 2022; and the final balance by April 1, 2022). I also understand that if I pay by paper check, each payment deadline is 15 days earlier: December 17, 2021; February 13, 2022; and March 17, 2022.*

Travel Insurance

I understand that I will be required to provide proof of travel insurance coverage for all members of my party as soon as possible but no later than April 1, 2022.*

Liability Waiver

I confirm that each member of my party is participating voluntarily and at her/his/their own risk and assumes all related risks, both known and unknown, of her/his/their participation. I understand that IKAR and/or its partners may require each member of my party to sign a liability waiver in order to participate.*

No Claim Guarantee

I understand that IKAR is making arrangements on my behalf for travel services and that in the case a covered event occurs for which one or more members of my party files a travel insurance claim, IKAR will make best efforts to provide necessary documentation related to my payments but cannot guarantee the outcome of any claims process.*

Delay Costs

I understand that this is group travel across multiple countries, and that all costs associated with delays caused by my party, including but not limited to transportation to catch up to the group, are my responsibility.*

Vaccination Status

I understand that I will be required to provide internationally valid proof of full COVID vaccination and all authorized boosters for every member of my party as soon as possible but no later than April 1, 2022.*

COVID Expenses

I understand that all COVID-related expenses, including but not limited to vaccination documentation, testing, and treatment, including travel delays and expenses related to quarantine and/or treatment) are my responsibility.*

Trip-Related Use of Personal Information

I understand that names, contact information, and photos of the members of my traveling party may be shared with other members of the trip and with trip providers & partners for purposes and uses directly related to the trip. I agree not to share other people’s personal information outside the trip without their permission.*
