Perkins Basic Application Cover and Certification Forms Submission

 

INSTRUCTIONS

Applicants must upload copies of the Application Cover Page, Certification Regarding Lobbying, and Federal Funding Accountability and Transparency Act (FFATA) Certification as part of the application on or before 11:59 p.m., CDT on June 30.


The following documents need to be uploaded:


  1. Application Cover Page
  2. Certification Regarding Lobbying
  3. Federal Funding Accountability and Transparency Act (FFATA)
  4. Certification of Compliance with Section 504 and ADA Title


Also: SAM.gov Registration (must be Active with an October 2025 or later expiration date)


Completion of these document uploads is necessary for your application to be considered complete.


The authorized representative of the applicant should sign these documents, and signatures may be in either hard copy or electronic form.


Remember the Local Application must be submitted through the Perkins Portal.

 
 
 
  1. To upload a file, go to the "browse files" tab.
  2. Locate the file on your device that you wish to upload.
  3. Choose the file from your device and then click the open/insert tab.
  4. The file you selected will appear in the file upload box.


Please note: You can upload a max of 10 documents at a time.
