Suspension Reinstatement

Please note that administrative suspensions for outpatient encounters only affect scheduling abilities. Providers will still be able to chart and close encounters, even when suspended for outpatient encounters.


This form will notify the Patient Access team to reinstate scheduling privileges for the provider noted below. Reinstatement can only occur during business hours - please allow 30 minutes after submission for reinstatement to occur.
