2. LGPAT Application Payment Receipt
3. 2 x 2 Photograph (Passport sized)
4. Official Transcript
5. Art Therapy Credential Board Exam (ATCBE)
6. Copy of MLA Certificate of Completion
7. Criminal History background Check
