Use this form to apply for a Military tuition waiver. Please carefully read all instructions. An approval request will be sent to the Veteran Coordinator to review your application. If your information is incorrect, you will receive an update request from Smartsheet requesting an update. Resubmit a waiver only if you have had a gap semester (Fall or Spring) or if you are switching from an undergraduate to graduate level waiver.
