Use this form to apply for a Military tuition waiver. Please carefully read all instructions. An approval request will be sent to the Veteran Coordinator to review your application. If your information is incorrect, you will receive an update request from Smartsheet requesting an update. Resubmit a waiver only if you have had a gap semester (Fall or Spring) or if you are switching from an undergraduate to graduate level waiver.

Waiver Information and Required Documentation

Select the applicable waiver options and note both the limitations for each waiver and the required documentation that needs to be uploaded. These waivers are a State Benefit, not a Federal Benefit, therefore the student MUST be a Washington Resident; for the Spouses and Dependent waiver, the Sponsor MUST also be a Washington Resident.

Resident Regular Tuition Limitations

Graduate & Professional Students: Value is based on the cost of resident regular graduate tuition and not on higher tier tuition rates for programs such as Nursing, Pharmacy, Veterinary Medicine and Medical.

The sponsor is deemed 100% disabled or paid at the 100% rate, in order to qualify for the state waiver.

Sponsor’s VA disability letter (We only need to verify the rating, feel free to omit or redact the medical description and pay amounts)*

Proof of student’s relationship to sponsor (birth certificate for children, birth certificate & marriage license for step children, marriage license for spouses or DEERS page showing authorized dependents)*
